USA HOCKEY REGISTRATION IS REQUIRED Follow the instructions listed below.  Your registration will not be accepeted without the USA Hockey Barcode attached from your USA HOCKEY REGISTRATION.

 

Instructions for Online Registration with USA Hockey

It is mandatory that all players register on-line with USA Hockey.  Players should attach their printed registration form and USA HOCKEY barcode when dropping off registration or mailing in the registration. The on-line registration process is simple and should take you no longer than 5 minutes to complete.

1.      Go to www.USAhockey.com

2.      Click on the quick link in the red box that says "Become a USA Hockey member"

3.      Simply follow all of the prompts to register.  Make sure to use the full legal name (no nicknames or shortened version of the name)

4.      You will need a credit card to make payment of $35 ( $30 USA Hockey $5 WAHA fees)

5.      At the end of the on-line registration process it will ask you to open confirmation and print. This is the page you will need to send in with your registration!!

I do hereby release and discharge the and Cornerstone Community Center Inc. and it's officers, agents, volunteers and staff from all claims resulting in injuries or damage and losses due to my participation in the activities provided by these organizations.
